Drop Off Form
If you plan to drop off your pet with us to receive treatment, please complete the drop off form, you can either bring the completed form with you at the time of your visit, fax the completed form in advance to (404) 881-6220, or email the form to intownah@gmail.com. It will help us expedite your check-in. If you have any questions about the form, we will be happy to go over them with you when you come in. Drop offs are accepted between 8:30am and 10:00am.
